At the beginning of this module, you will learn how housing and social conditions shape health outcomes, and why they must be understood as central components of integrated health and social care (IHSC). You will explore how pathways out of homelessness are not only medical, but deeply social—rooted in stability, rights, and participation.
You will realize that without addressing housing, income, legal status, and community connection, health interventions alone cannot lead to sustainable change. This module will help you see that integration begins where services extend beyond treatment and engage with the broader conditions of people's lives.
YOU WILL DEVELOP THE SKILLS TO
Explain the relationship between housing and health as a key social determinant,
Evaluate the evidence and principles of Housing First (including the European framework) and consider their contextual adaptation,
Distinguish between Housing First and other housing-led models,
Identify pathways to social reintegration, including access to benefits, employment, and broader participation,
Understand the role of legal support, community organisations, and NGOs in sustaining integration,
Recognise the value of peer involvement and empowerment, and map local support ecosystems and identify gaps in integration.
Through this module, you will begin to understand integration not only as coordination between services, but as a process of reconnecting people to housing, rights, and community
Ultimately, this module invites you to reflect:
If housing is a foundation for health, what does it mean for care systems that do not secure it?
