Principles of Integrated Health and Social Care
Why Integrated Care Matters
People experiencing homelessness rarely present with a single problem.
They may live with:
Chronic disease
Mental health conditions
Substance use
Lack of housing
Administrative barriers
Legal issues
Yet systems are typically organised in separate silos.
Health services treat illness.
Social services address benefits.
Housing systems manage accommodation.
Justice systems handle legal issues.
When these systems do not communicate, individuals fall between them.
Integrated care exists to prevent this fragmentation.
Where the Idea Comes From
The idea behind integrated care is simple:
Care should be organised around people — not around institutions.
Since the late 20th century, healthcare reforms have increasingly recognised that fragmented services produce poor outcomes, especially for people with complex needs.
Integrated care emerged as a practical response:
Improve coordination
Reduce duplication
Ensure continuity
Connect health with social determinants
It is less about theory and more about redesigning systems to work coherently.
