Four Core Principles of Integrated Health and Social Care
Drawing from the Health Care for the Homeless (HCH) model (USA), four foundational principles define effective integrated care for people experiencing homelessness.
To strengthen clarity within the European IHSC context, we use adapted titles that remain faithful to HCH while making their meaning more explicit.
| Original HCH Principle | IHSC Adapted Title | Core Meaning |
|---|---|---|
| Comprehensiveness | Whole-Person Integrated Care | Addressing the full health and social reality of the person |
| Accessibility | Low-Barrier and Equitable Access | Removing structural and practical obstacles to care |
| Attitude | Trust-Based and Trauma-Informed Practice | Relational, respectful and non-judgmental care |
| Philosophy | Health and Housing as Rights – System Responsibility | Social justice, dignity and long-term stabilisation |
These principles translate integration from theory into operational practice.
