Variation Across Contexts
Across Europe and globally, readiness for integrated care varies significantly.
Traditional healthcare systems are often organised around single-disease specialisation.
SOME REGIONS HAVE:

Established multidisciplinary structures.

Formalised intersectoral agreements.

Data-sharing frameworks.

Housing-led strategies linked to health services.

OTHERS RELY ON:

Project-based initiatives.

NGO-driven coordination.

Informal professional relationships.

Short-term funding cycles.

Many systems are somewhere in between. Recognising this diversity is essential.
Integrated care cannot be copied mechanically from one context to another.
It must be adapted to local governance, resources and institutional cultures.
