From Fragmentation to Coordination
In fragmented systems, people experiencing homelessness must navigate complexity on their own.
• They repeat their stories across services.
• They move between emergency departments, shelters and temporary housing.
• They experience interrupted treatment.
• They fall through gaps created by institutional boundaries.
Integration aims to reverse this dynamic.
It seeks to create:
Continuity instead of episodic intervention.
Coordination instead of parallel services.
Stability instead of crisis cycling.
Long-term pathways instead of short-term responses.
But achieving this shift requires readiness.
Not every system is equally prepared to integrate.
