Institutional and Structural Barriers to Implementation
Even when policy frameworks support integrated care, implementation often remains limited. The gap between strategy and practice is rarely caused by a lack of professional commitment. More often, it is the result of structural barriers embedded within institutions.
1. Sectoral Fragmentation
Health systems, social services and housing sectors frequently operate under:

Separate ministries or departments

Distinct governance structures

Independent funding streams

Different accountability frameworks

Each sector has its own priorities, reporting systems and performance indicators.
As a result:
HEALTH SERVICES
focus on clinical outcomes.
SOCIAL SERVICES
focus on eligibility and compliance.
HOUSING AUTHORITIES
focus on tenancy management.
Integrated outcomes — such as stability, continuity and long-term reintegration — often fall between sectors.
Fragmentation is therefore systemic, not accidental.
