Ethical Tensions in Complex Mental and Physical Health Care
Integrated care becomes most demanding when clinical complexity intersects with ethical uncertainty.
In mental health, addiction, chronic illness, and homelessness, ethical dilemmas are frequent — not exceptional.
These dilemmas often arise at the point where:
Autonomy meets risk
Harm reduction meets abstinence expectations
Parenting meets safeguarding
Chronic pain meets addiction
Relapse meets housing stability
Worker safety meets client dignity
Ethical maturity is a marker of integration maturity.
Each tab below explores one ethical tension in complex care — read through all six to continue.
1. Autonomy vs Protection
Consider Peter: PTSD, Opioid dependence, chronic pain, repeated overdose risk
He refuses inpatient treatment.
Questions arise:
Do we respect refusal?
Do we pursue involuntary admission?
What constitutes capacity in intoxication?
Integrated care requires balancing:
Respect for autonomy
Duty of care
Proportional response
Non-maleficence
The principle is not control — but proportionality.
