The SCIROCCO Dimensions
The model includes 12 key dimensions that collectively describe system readiness for integrated care.
Below is a concise overview.
1 · Readiness to Change
The degree to which stakeholders recognise the need for integration and are willing to engage in reform.
2 · Structure and Governance
The clarity of roles, responsibilities and coordination mechanisms across sectors.
3 · Information and eHealth Services
The availability and interoperability of digital systems supporting information sharing and coordination.
4 · Standardisation and Simplification
The extent to which processes, pathways and protocols are aligned and simplified across sectors.
5 · Funding
The alignment and sustainability of financial mechanisms supporting integrated models.
6 · Removal of Inhibitors
The identification and reduction of regulatory, organisational or cultural barriers to integration.
7 · Population Approach
The extent to which services are designed based on population needs rather than isolated service provision.
8 · Citizen Empowerment
The degree to which service users are involved in decision-making, planning and evaluation.
9 · Evaluation Methods
The existence of monitoring systems and performance indicators supporting integrated outcomes.
10 · Breadth of Ambition
The scope and scale of integration — whether limited to pilots or embedded system-wide.
11 · Innovation Management
The capacity of the system to develop, test and scale new integrated solutions.
12 · Capacity Building
The development of workforce skills, training and competencies required for integrated care.
Why These Dimensions Matter for IHSC
For people experiencing homelessness, integration requires alignment across all of these areas.
For example:
Without funding alignment, housing-health integration remains fragile.
Without data systems, continuity of care cannot be monitored.
Without governance clarity, responsibility becomes diffuse.
Without citizen empowerment, services risk being paternalistic.
Without capacity building, integration remains theoretical.
Weakness in one domain can undermine progress in others.
Integration maturity is therefore multidimensional.
