Why System Readiness Matters
Integrated care improves outcomes for people experiencing homelessness. Evidence consistently shows that coordinated approaches:
Reduce avoidable hospital admissions.
Improve continuity of care.
Support housing stability.
Reduce long-term system costs.
Yet despite this evidence, integrated care is not universally implemented.
The reason is not lack of knowledge.
The reason is structural complexity.
Health systems, social services, housing authorities and municipalities often operate under:

Separate governance structures.

Distinct funding streams.

Different accountability frameworks.

Incompatible data systems.

Divergent professional cultures.

Fragmentation is not accidental. It is embedded in how systems are designed.
Integrated care therefore requires more than good practice.
It requires structural alignment.
